USA HOCKEY Membership Insurance
All players are required to submit a copy of their USA Hockey receipt with their league registration. Players not registered with USA Hockey for the 2010-2011 Season will not be allowed to participate in the league.

USA Hockey has changed their registration dates for annual registration. In the past the USA Hockey season ran from Sept 1 through Aug 31. The new season now runs from May 1 through April 30 of the following year. All players must register with USA Hockey for the spring and summer season and that registration will carry through the fall winter 2010-11 Season regardless of what team or association you play/coach for.www.usahockeyregistration.com.
Team coordinators take full responsibility for their team. Coordinators are expected to collect all registration forms and payments, enforce all SSHL game rules and suspensions, and communicate all information regarding League Standard of Play.
Team Payment: $1800/Team
Team Fee Option
You may choose to register/pay as a team. The team fee is $1800 but is only available to teams paying in full with one check or credit card prior to the beginning of the season. We will not accept multiple checks or payment plans if you are using the “Team Fee” option. If you pay as a team there is no minimum number of players required for your roster and the DPSC will not add players to your roster. The maximum roster size is still 20 players.

It is the responsibility of the team coordinator to collect all registration forms and payment. Please do not have your players drop off individual payments at the rink. Players should give their payments to the Coordinator, who in turn will pay the total balance due at the time of team registration.
Individual Player Fee: $209/Player

1. Official Team Roster with jersey numbers
2. Completed Registration Form for each player with payment attached
3. Signed Waiver from each player
4. USA Hockey Receipt for All Participating Players (2010-2011 Season)
5. *FULL PAYMENT (Based on payment terms below)

Individual Payments for 4 on 4 Summer “A” League Season
In order to make it easier to track payments and provide the teams who choose to pay in a lump sum a significant discount here are the payment terms for the 2010 Summer 4 on 4 “ A” League Season. Every player participating will pay a fee of $209. The minimum number of players per team is 10 paid players. If you have less than 10 players on your team the FB-DPSC will add paid players at their discretion with the total number of paid players not to exceed 10. Once you have 10 paid players on your team you may add sub players up to a roster total of 20 players maximum. No team packets will be accepted unless accompanied by payments from each rostered player. Teams of less than 10 players will not be admitted unless they choose to pay the Team Fee Option.
Game Format
► 3-minute warm up
► Each period will be thirteen (13) minutes in length, no icings will occur
► Run time occurs when:
7 goal or larger lead in the 3rd period
15 total penalties occur during the game regardless of period
► No game shall end in a tie. If a game is tied at the end of regulation then a 5-minute (running clock) sudden death, 3 on 3 will be played. If the game remains tied at the end of OT, a 3-man shootout will used to determine the winner. If the game remains tied, a sudden death shootout will be used to determine a winner.
** No player who was serving a penalty at the end of the overtime period may participate in the shootout. All players must shoot once before a player shoots for a second time.
** Penalties: Power plays will be 4 on 3, if a second penalty is called on the team that is down a man then it will go to 5 on 3 until the first penalty expires and the first whistle after the penalty expires it will revert to 4 on 3.
